Requests for information for reconsideration should be directed to:

Requests for reconsideration should be accompanied by a letter stating:

• what information was requested;
• when the information was requested;
• what the reply was to the request; and
• why it is felt the request should be re-evaluated or why it is the request was treated unfairly.

Requisitioners should attach copies of any relevant correspondence. There is no charge for making a complaint to the Disclosure Committee.
